In the state of Iowa, you do not need a referral to receive Occupational therapy. However, if you are billing your insurance, your insurance may require you to obtain a physician’s referral.

I am an in network provider for Wellmark/BCBS and The VIP Network. I take HSA/FSA and private pay.

The therapist needs to be able to see your body during treatment, so please follow these guidelines:

1. Women: Please wear under garments or tank and loose fitting athletic shorts.

2. Men: Please wear undergarments or loose fitting athletic shorts.

3. No lotions, topical ointments, or oils on areas of treatment. Lotions and make-up provide too much slip, preventing the therapist’s hands from performing the holds properly.

4. Bring an open mind and an open heart!

Cancellation Policy: I generally have a list of clients who would like to get in earlier, so I appreciate at least 24 hours notice if you cannot make your appointment. This allows me sufficient time to contact those who need services and keep my schedule full. 

1. This system provides text and email reminders based on the information you put in.

2. If you do not cancel before the 24 hour notice, you will be asked to pay for the missed visit in full.
